What should I bring?
Please bring the following to your first visit
- Dental insurance card(s), if applicable
- Medical insurance card(s), if applicable
- Completed registration forms (if not already completed online)
- Health History Form *if you require premedication prior to dental cleanings please inform us prior to the appointment
- Current medication list
- Recent dental x-rays (within the past year) may be forwarded by the previous dental office prior to your first appointment for review
New Patients
We welcome new patients! At your first appointment we will perform an exam to assess your needs. Whether you have a broken or painful tooth, or are just searching for a new dental home, we are glad you found us and look forward to meeting you!
